Formating to use in the Mass Elite Player Profile

PRINT FOR REFERENCE WHEN COMPLETEING YOUR PROFILE

*Most important throughout:
DO NOT USE COMMAS
USE SEMI COLON “;” TO SEPARATE EVENTS, ETC…

Complete your profile as if writing a school paper, use CAPITAL letters on all names and beginning of sentences, use spaces separating a number and a word, for example: 19 Pine View Dr

Any information NOT AVAILABLE put “n/a” not N/A, not NA, not na. 

Please follow formatting exactly so that the profile CD looks professional, we do not edit this information. 

Phone/ cell numbers: Do not put a 1.  Include area code.  Use dashes between area code, exchange and number not parenthesis.  Ex: 508-234-6789

e-mail: all LOWER CASE.  If have two e-mails use a ; (semi colon) between them.  NOT A COMMA. 

Date of Birth:  enter as mm/dd/yyyy

Weight: Ex: 160 lbs  Include “lbs”

Guardians: use “and” between first names or between two different names.  DO NOT USE A COMMA.  Do not use a “&” or “+” .Example: Lisa and David Orecchio or Lisa Johnson and David Orecchio 

Years Experienced: enter # only

Statistics: if not available put “n/a” not N/A, not NA, not na.  Enter # only.
